In California, Anthem Blue Cross is the trade name of Blue Cross of California. Anthem Blue Cross and Anthem Blue Cross Life and Health Insurance Company are independent licensees of the Blue Cross Association.
There are a variety of anthem rehab insurance coverage plans in California organized into tiers. They range from Bronze tier, with a low monthly premium but extremely high deductible, to Platinum tier, with an extremely high monthly premium but very low deductible (or no deductible). The Silver and Gold plans comprise the middle of this tiered system.
